The past decade has seen an upsurge in global
interest of various aspects pertaining towards
enhancement of food safety and security. With
increasing knowledge of food safety, the world
is witnessing tremendous efforts in improving
the well-being of mankind. A rise in food safety
concerns is a direct reflection of major global
awareness in agro-foods sectors in world trade.
Several recommendations have been made by
various governing bodies and committees to
solve food safety issues, which are all mainly
aimed at benefiting consumers. In the present
world scenario, note that that economic loss and
instability due to food safety issues can have a

For the 57th Nestlé Pediatric Nutrition Workshop, which took place in May
2005 at Half Moon Bay, San Francisco, the topic ‘Primary Prevention by
Nutrition Intervention in Infancy and Childhood’ was chosen. Early nutrition
seems to be involved in the mechanism of control, especially taking into
account the role of protein and long-chain polyunsaturated fatty acids (LCPUFAs).
It seems that the new generation of infant formulas already takes
those findings into consideration.

This volume was inspired by a symposium, ‘Everyday ethics for the food scientist: Ethics
in research, education and the workplace’, co-sponsored by the student programming
and the education divisions of the Institute of Food Technologists at the 2009 Annual
Meeting. Additional topics and authors with an international perspective have been
introduced to produce a comprehensive treatment that, it is hoped, will serve a wide
audience.
Ethical considerations are involved in every aspect of a food professional’s education
and career, but the subject is rarely taught explicitly. Unfortunate examples of ethical
lapses abound in news reports. Large issues of strategy have ethical components, but
guiding principles are sometimes difficult to articulate.
Some business and engineering colleges have adopted an emphasis on values and
ethics, but these need to be communicated to food professionals who have the opportunity

The time is now for this textbook on primary care nutrition. Had it been written at any time during
the last 30 years, it would have been thought to be totally irrelevant and an unnecessary diversion
from the serious work of primary care practitioners. How could nutrition be important to a medical
profession schooled in drugs and surgery for the prevention and treatment of disease?
Nutrition was an afterthought for most physicians that had nothing to do with medicine

Since global recognition of the dietary guidelines that include increased consumption of
plant-based foods for the prevention of chronic diseases and maintaining good health,
there has been a considerable interest in the biologically active compounds that are
present in plant foods. These compounds have been referred to, among other terms, as
‘Phytochemicals’, ‘Phytonutrients’, ‘Nutraceuticals’ and ‘Functional ingredients”. They
include a multitude of compounds having different chemical identities, biological
activities and mechanisms of action. The scope of ‘Phytochemicals’ has expanded
beyond their initial applications to food to include therapeutics, pharmaceuticals and
cosmeceuticals. Although advancements have been made in the field of phytochemicals
in the past few decades, more information on the analytical methods of isolation and
characterization, their occurrence, biological activity, mechanisms of action and
applications to the food and other health industries need to be obtained through
systematic scientific investigations. Recognizing this need for more information, a plan
to publish a book that brings together up to date information on various aspects of
phytochemicals was initiated. This book is a collection of several articles that range in

Nutrition has the primary goal to maintain, or if possible to improve, health.
Pharmaceuticals, on the other hand, are generally developed to treat, cure
or to prevent diseases [1]. In the last few years the disciplines of pharma and
nutrition have evolved separately. Nonetheless, worldwide increased incidence of
complex multifactorial disorders, particularly chronic and degenerative diseases
and their growing burden in modern society have narrowed the gap between
pharmacology and nutrition science [2]. Hence, multifactorial diseases probably
require multi-pathway understanding and multi-targeting approaches which might
lead to compound combinations [2]. This rediscovered common ground between
the complementary values of pharma and nutrition can be conceptualized in the
term “pharma-nutrition.” In this respect, functional foods or dietary supplements,
i.e., those that carry substantial health claims, can be helpful in reducing health risk
factors and may thereby prevent chronic diseases [1]. Various chapters in this book
highlight the aspects of molecular characteristics of food ingredients towards
clinical effectiveness and relevance.

Humans, like all mammals, have an inborn desire to nurture and suckle their young
and the act of feeding is important for the bonding between mother and child. The birth
of a critically ill preterm infant disrupts this, but for a mother, the use of her milk to
feed her infant may be the most tangible role she has in the medical care of her critically sick child. Parents and caregivers see growth and feeding as important milestones
first demonstrating increasing stability, then signaling the start of recovery, and
finally showing readiness for discharge home.
Despite our intuitive and emotional connection with growth, preterm infants grow
poorly after birth and very commonly develop ex utero growth restriction (EUGR)
or postnatal growth failure. There are many reasons for this including the associated medical conditions of prematurity, but inadequate nutrient intake plays a large
part. This results both from technical difficulties in providing adequate nutrition,
and from fears about the complications associated with doing so, including metabolic derangements such as hyperglycemia and hyperlipidemia, and diseases such as
necrotizing enterocolitis.

The first question we often ask athletes when they enter our office is out of 100 percent,
how much does nutrition affect their performance? Typically, most athletes are kind
enough to give nutrition about 10 to 15 percent and attribute the rest to training and
recovery. This response tells us a great deal about how these athletes are viewing their
food. When we point out that an athlete’s nutrition should be designed to support
training and enhance recovery, athletes realize that nutrition can affect performance
100 percent.
Timing nutrient ingestion is a concept that has become increasingly popular in
the world of sport performance. When the primary goal between training sessions
is to recover, nutrition becomes a key component that will allow the body to adapt
to the imposed training demand; thus nutrition to support the recovery process and
adaptations to training must start before training even begins. This concept might best
be understood by viewing the body as a furnace and its energy supply as the fuel for
the furnace. The furnace in a house functions best when we ensure that enough fuel
is readily available, and a steady supply of fuel to the furnace keeps it functioning
most efficiently. The furnace also runs best when we consider the type of fuel we use
to maintain the fire. A high-quality fuel will ensure that the furnace runs most effectively, whereas low-quality fuel can lead to eventual breakdown of the system. In the
same sense, if an athlete does not fuel the body at regular intervals throughout the day
and provides the body with poor food choices for the energy demand it is under (i.e.,
training load), then recovery from training cannot occur and adaptation cannot take
place. The furnace is equal to the engine of the human body, which is also known as
metabolism. The rate of metabolism is dependent on consistently needing to operate the body within an optimal range. If the body is deprived of food, metabolism is
reduced to compensate, and over time the body will operate continuously under this
reduced rate. When quality food such as whole grains, fruits, and protein is provided
steadily throughout the day, metabolism increases and thus helps minimize fat stores
while improving muscle mass.

Each year, millions of children throughout the world are subjected to the life -
threatening ravages of malnutrition. The nutritional issues that children face
today have radically changed as knowledge on disease processes as well as their
treatments have improved.
We know the effect of nutrition is profound. Good nutrition not only assures
proper growth and development, but it also enables each organ system to do
its job properly. In ill children, nutrition is paramount, affecting both morbidity
and mortality. As medicine has specialized, the nutritional care of children has
also become increasingly specialized. This handbook serves as a broad guide
to the nutritional care for the developing infant and child as well as for children
with specialized medical needs.
This handbook is the fruition of a long - held dream by many at Seattle
Children ’ s Hospital to standardize the nutritional care of our patients.
Collectively, it represents years of experience and specialized knowledge in the
nutritional assessment and care of infants, children, and teens.

There is no other time in life when the provision of
adequate and balanced nutrition is of greater importance than during infancy and childhood. During this dynamic phase of life characterized by
rapid growth, development and developmental
plasticity, a sufficient amount and appropriate
composition of substrates both in health and disease are of key importance for growth, functional
outcomes such as cognition and immune response,
and the metabolic programming of long-term
health and well-being. While a number of excellent
textbooks on pediatric nutrition are available that
provide detailed accounts on the scientific and
physiologic basis of nutrition as well as its application in clinical practice, busy physicians and other
health care professionals often find it difficult to
devote sufficient time to the elaborate and extensive study of books on just one aspect of their practice. Therefore, we developed this compact reference book with the aim to provide concise information to readers who seek quick guidance on
practically relevant issues in the nutrition of infants, children and adolescents.

The role of nutrition in the well-being of infants and children has expanded
beyond determining the quantities of nutrients needed to prevent classic
defi ciency states such as scurvy. Modern nutritional science now seeks to
understand the interaction between nutrition and human physiology at the
level of genetics and molecular biology. In addition, a major direction in nutritional research is to examine how those interactions that occur early in life
affect health and development not only at different periods during infancy
and childhood but also over the life span of the individual. To provide one
example, does the provision of specifi c nutrients such as long-chain fatty acids during pregnancy or during early infancy infl uence cognition, behavior,
and vision of the young infant, and if so, do those benefi ts persist into later
childhood and even adulthood? We also have come to recognize that nutrients, foods, and eating patterns are not synonymous. What and how infants
and children are fed and eat have many determinants, including those that
are cultural, traditional, behavioral, cognitive, and environmental, and this is
another major current focus of investigation.

The provision of optimal nutrition to the critically ill child,
to offset the catabolic effects of the illness or injury and to
enhance clinical outcomes, is an important objective for
healthcare providers. The last decade has seen a resurgence
in interest in this area of critical care, with an increasing
number of research publications and consensus documents. The field of critical care nutrition has evolved, and
clear associations between bedside nutrient delivery and
outcomes have been demonstrated. Nutrition delivery is
no longer just supportive care, but it is now recognized as
an important therapy with nutrients that can modulate disease. Despite the enthusiasm and the explosion in research
on the subject, there are many unanswered questions, and
the quest for best practices remains elusive, especially in
the pediatric intensive care unit (PICU). The individual
practitioner—the trainee, the intensivist, the dietitian, the
nurse—is left with a number of practical questions on the
nutritional management of the individual child in the PICU.

We still don’t know all the various factors that intertwine with and influence
nutrition. Often, we assume that poor nutrition is a socioeconomic issue, but
the articles in this compendium make clear that this simplistic assumption is not
accurate. This is a massive, multifaceted topic. The articles included here were
chosen to present as accurate a total image as possible, based on recent research.

This edition has been updated with new references and information in every chapter, but it also includes chapters that are entirely new. For example, many athletes face chronic or acute GI problems that interfere with training and performance. Included in this edition is information to help athletes understand the possible causes of GI distress and strategies for alleviating these problems. Chapter 5 includes material on gluten intolerance (celiac disease), which is increasingly prevalent in both athlete and nonathlete groups. This new edition also includes evidenced-based research findings on nutrition strategies for reducing muscle soreness, an area that has recently received a great deal of scientific attention but for which there remains an enormous amount of misinformation.

Development of the World Health Organization, as part of its efforts to promote the integration of evidence-
based public health interventions in basic training of health professionals. The Model Chapter is designed for use
in textbooks used by health sciences faculties, as a result of the positive experience with the Model Chapter on
Integrated Management of Childhood Illness.

How did breastfeeding become so unnecessarily complicated? As the
years go by, I am often astounded by how we manage to add new
wrinkles of complexity to something that should be easy, natural, and
uncomplicated. Of course, breastfeeding itself has not become more
complex than it was, say, 200 years ago. Rather, it is the way we think
about breastfeeding that has changed.
