Nutraceuticals and functional foods are becoming increasingly popular worldwide.
The “Ayurvedic Medicine” from India and the Chinese Traditional Medicine have
demonstrated the therapeutic efficacy of a number of botanical or herbal medicines
for thousands of years. The Unani (or Yunani) system of medicine is a Persian Arabic traditional medicine practiced in India (1526–1707 AD) and South-Central Asia,
which was based on the teachings of the Greek physician Hippocrates. In recent
years, there have been significant advances in the development of pharmaceuticals
and drugs aimed at treating a variety of disease conditions. However, most of these
pharmaceuticals or drugs are expensive and are associated with a number of adverse
side effects. More recently, a significant number of nutraceuticals and functional/
medical foods have been introduced to the worldwide marketplace, backed by broadbased safety and efficacy studies. These natural products are often less expensive,
and are available as over-the-counter products and have therefore gained popularity. To keep up with these changes medical and allied-health schools are training
physicians and health-care professionals on the use and benefits of alternative and
complementary medicine.

many years I have witnessed those in my community and clinics being victimized by socioeconomic
forces ultimately resulting in obesity and its untoward manifestations. As a teen in the mid-1970s, I
found myself severely overweight, but back then I was a rarity and was quite easily stigmatized by
society. My personal experience with Eastern principles through martial arts coupled with diet and
lifestyle modifi cations was highly successful. This “integrative” approach toward achieving effective
weight loss motivated me to help others fl ourish. As a practitioner of integrative medicine for many
years, I have witnessed an ever expanding literature which supports the application of principles I
experienced as a young man to manage weight disorders and its adverse consequences; however, a
comprehensive resource guide for healthcare practitioners was lacking.

The epidemic of hypertension affects about 1 billion people worldwide.
In the United States alone, some 90 million people have this disease. It is
a major risk factor for cardiovascular disease, stroke, and kidney disease.
Although it is mostly a disease of the elderly, with current trends toward
obesity, sedentary lifestyles, and poor diets, it is increasingly becoming a
disease of the young too. An ever-increasing portion of medical budgets
is allocated to its treatment, often making many drug companies quite
profitable. Clever advertising and prudent public relations are the driving factors in the popularity of many drugs, despite the fact that most
other drugs of the same class have near indistinguishable efficacy and
side effects. Yet, despite the apparent popularity of many drugs, no patient
really wants to take them. In fact, most patients, especially the younger
ones, bargain and plead for more time or alternative treatments. Initially,
medication is resisted by everyone, and for good reason, as it usually leads
to many others along with dreaded side effects

healing modalities and emphasizes natural and traditional approaches and favors
noninvasive low-risk interventions. The movement of integrative medicine dates
back several decades to a more evolutionary process whose growth was organic
and a number of pioneers who collectively increased public awareness and ultimately popularity. Due to consumer demand of integrative medicine, congressional legislation established the Office of Alternative Medicine in October 1991 and
appropriated $2M for its inception under the auspices of the National Institutes of
Health (NIH).* The goal was to fund research studies to confirm the validity of
integrative medicine healing modalities. Despite economic fluxes, there has been
a steady growth in funding for this office whose name was changed in 2000 as the
National Center for Complementary and Alternative Medicine (NCCAM), which
was refined as the federal government’s lead agency for scientific research on
the diverse medical and health-care systems, practices, and products that are not
generally considered part of conventional medicine. Ultimately, the goal of the 65
FTE-staffed agency is to advance the science and practice of complementary and
alternative medicine and promote evidence-based personalized strategies while
enabling practitioner’s data to incorporate evidence-based strategies to promote
wellness and health care.† NCCAM and philanthropic funding of research encouraged research and rapid growth of integrative medicine with ample evidence supporting its benefits. In 1994, Dr. Andrew Weil, philanthropists, and colleagues
formed the nation’s first university-based training program in integrative medicine at the University of Arizona. In 2001, a conversation began the formation of
The Bravewell Collaborative, a community of philanthropists that are devoted to
the growth of integrative medicine and partnered with leading academic physicians to transform health care into a more humanistic integrative model.‡ The
Bravewell Collaborative then brought together over 50 major academic centers
in the United States to form the Consortium of Academic Health Centers for
Integrative Medicine. In 2014, The American Board of Integrative Medicine came
together under the American Board of Physician Specialties to provide the public
a means to recognize physicians who are qualified in the practice of integrative
medicine

The perception of food quality is an enormously complex subject that, in
spite of many significant advances made over recent years, we are only
beginning to understand. Take the many varied physiological inputs that
are generated by the human sense organs when consuming food, couple
these with the psychological and neurological factors that modify these
sensations in the brain, and it is clear that the human perceptual process is
extraordinarily difficult to mimic by any instrumental methods, however
sophisticated.

People have integrated nutrition, food science and cooking since the beginning of time—without
even knowing it. The first foods and beverages were chosen to stay alive. Ancients ate meat and vegetation
for sustenance and drank water for hydration. Little did they realize that the substances in meats and
plants (proteins, fats and carbohydrates) are nutrients that are responsible for energy, strength and
well-being.

Traditionally, the overall aim of food processing has been to provide food that is safe
while maintaining quality during its shelf-life. More recently, however, this objective
has evolved. Consumers are now demanding “next-generation” processed foods from
fresh ingredients, with high nutritional balance, health-promoting components, and
assured food safety. Food quality attributes, such as flavor, aroma, texture, appearance,
and nutritional content are strongly dependent on the way the food is processed. The
food industry is becoming increasingly competitive and dynamic, and strives to
develop high-quality, freshly prepared food products. In trying to achieve this objective, food manufacturers are today presented with a growing array of new technologies
that have the potential to improve, or replace, conventional processing technologies to
deliver higher-quality and better consumer-targeted food products, which meet many,
if not all, of the demands of the modern consumer. These new, or innovative, technologies are in various stages of development, including some still at the R&D stage, and
others that have been commercialized as alternatives to conventional processing
technologies.

As in the previous editions of this CRC Desk Reference for Nutrition, terms of interest to the nutritionist are listed alphabetically. Included are medical terms, food science terms, metabolic terms,
physiologic terms, drug terms, biotechnology terms, and nutrition terms. Since nutrition and foods
are integrated sciences, not all of these terms will be useful to all readers. However, there should be
a large number of terms useful to many readers. The third edition provides many more terms than
were provided in the earlier editions. There have been some notable changes and inclusions that
hopefully will make this edition more useful than the earlier ones. A web address has been included
to give the reader access to the extensive tables of food composition maintained by USDA. In addition, a web address for dietary reference intakes (DRIs) has been included to again provide the most
current recommendations for nutrient intakes. These recommendations are in a state of flux. As the
information base expands with respect to nutrient use and need, the DRIs are changed to reflect this
newer knowledge. The recommended dietary allowance (RDA) table found in the first edition has
been omitted, as have the many tables of food composition.

Abstract: This chapter describes what is known about the main components and
responses of the innate and adaptive immune system of fi sh. The chapter fi rst
reviews the organs, cells and molecules of the immune system known in a few
economically important or model fi sh species. Molecular evidence suggests a
similar immune system exists throughout the jawed vertebrates yet marked
differences are also apparent. The innate parameters are at the forefront of fi sh
immune defence and are a crucial factor in disease resistance. The adaptive
response of fi sh is commonly delayed but is essential for long lasting immunity
and a key factor in successful vaccination.

The latter is only found in vertebrates while the former is an ancient system that goes
back in evolution to insects and plants.

We are very pleased to present the Proceedings of the International Symposium on Food and
Nutrition Security: Food-based Approaches for Improving Diets and Raising Levels of Nutrition. The
symposium, organized by the Nutrition and Consumer Protection Division of the Food and
Agriculture Organization of the United Nations (FAO), was held at FAO Headquarters, Rome,
Italy from 7 to 9 December 2010. A significant number of authors from different backgrounds
and experiences contributed to the 35 papers that were presented and provide a range of
views and analyses that form a rich body of knowledge and experience.

Growth is universally used by health care professionals and caregivers to
judge the well-being of babies and children, and this is based on an extensive
scientific literature linking aberrant growth of either extreme, failure to thrive
or rapid excessive growth, with adverse health and developmental outcomes.
On one hand, poor growth in early life is most commonly associated with poor
neurodevelopmental outcomes, while on the other hand rapid and excessive
growth has been associated with obesity and detrimental cardiovascular outcomes. While such statements do provide a simple summary, they assume
knowledge of optimal growth patterns and how these can be achieved. With
clear gaps in these areas, the challenge of this workshop was to explore in some
detail the associations of early growth patterns with later neurodevelopment,
obesity, cardiovascular outcomes and longevity in both industrialized and semiindustrialized societies.

Hypertension remains an important risk factor for premature cardiovascular
disease and accelerated atherosclerosis. It has become a major public health
hazard not only in the so-called industrialized nations, but also in the emerging
economies. Hypertension research is increasing rapidly and shedding more light
on previously unknown diagnostic and therapeutic aspects of this most prevalent
cardiovascular disease risk factor; consequently, the amount of literature published
is also growing at an unprecedented pace.

Nutritional science, biomedical science and food science are fundamental
components of the knowledge and understanding required of dietitians.

practitioner unless combined with highly developed communication skills.
A critical and applied understanding of communication is therefore a requirement

Hypertension and Stroke: Pathophysiology and Management reviews the pathophysiologic relationship between hypertension and cerebrovascular disease and the management
of blood pressure in a variety of settings such as primary stroke prevention, acute ischemic
and hemorrhagic stroke, secondary stroke prevention, and vascular cognitive impairment.
Hypertension is one of the most important global public health challenges, and there is a
close linkage between hypertension and cerebrovascular disease. Hypertension is the most
significant modifiable risk factor for cerebrovascular disease. The importance of adequately
treating elevated blood pressure in the primary prevention of stroke and management of
cerebrovascular disease is widely accepted. It is estimated that about 25% and up to 50% of
strokes could be prevented by blood pressure control. In the course of clinical practice, however, questions arise about the management of blood pressure in the acute, subacute, and
chronic phases of stroke. For example, when is it safe to initiate blood pressure-lowering
therapy after acute ischemic stroke? Is it dangerous to lower blood pressure in elderly persons who have had a stroke? What is the blood pressure-lowering target after a stroke has
occurred to maximize recurrent stroke prevention? Which blood pressure-lowering agents
are most efficacious and safe for persons who have had a stroke or for those in the general
population to prevent a first stroke? In this book we explore answers to these and many
more important aspects of hypertension and stroke. In recent years, several large clinical
trials which address blood pressure management for cerebrovascular disease have been
published. In this book we have attempted to collate and synthesize this rapidly expanding
knowledge base in a form that can be easily accessed and utilized by treating physicians.

Man, 70 years old, ex-smoker since 10 years (20 smoked
cigarettes for 30 years), and overweight after the retirement
(10 years ago), when his blood pressure began to increase.
Eight years ago, he began to take atenolol 50 mg in the
morning, and then it was interrupted because of bradycardia. Therefore, 4 years ago he began taking ramipril/hydrochlorothiazide 5/25 mg ½ tablet each morning. Lately, his
general practitioner (GP) added doxazosin 2 mg 1 tablet
taken at 2 o’clock in the afternoon, because of suboptimal
BP control. He usually does not care much about his health,
but he comes to our hypertension clinic for a general
checkup, hoping to reduce the number of tablets he takes
each day.

Nutrition is one of the most important requirements of humans, after oxygen and water. Human
evolution is closely related to nutrition and aimed
at improving nutrition to meet the high energy
needs of evolving brain and body. Globally about
50.5 million children are undernourished, 38.3
million overweight and 20 million obese. Poverty,
war, famine, sanctions and comorbidities are the
common causes of malnutrition. Even in the economically developed countries, malnutrition occurs
because of food faddism, isolation, depression
addiction and prolonged intensive care treatment.

Consumer preferences toward healthy food choices have driven the development of functional foods by contemporary food markets. Fermentation was
historically used to preserve food, but today it is gaining tremendous attention since it provides healthy food products that go beyond basic nutrition
and sensory properties. Existing scientific data show that many fermented
foods have both nutritive and nonnutritive compounds that have the potential to modulate specific target functions in the body, which contribute to the
health and well-being of consumers. Naturally fermented foods and beverages contain beneficial microorganisms that transform the chemical constituents of raw materials from animal/plant origin during food fermentation,
thereby improving the sensory, nutritional, and health-promoting properties
of the food. In particular, microorganisms in fermented foods facilitate the
bioavailability of nutrients, enrich the food with bioactive compounds, confer probiotic functions, degrade toxic components and antinutritive factors,
and impart biopreservative effects by producing antioxidant and antimicrobial compound

Three years ago, in May 2001, the World Health Assembly came to the
conclusion that it was safe for the majority of infants to be exclusively
breastfed for the first 6 months of life, and extending the period of exclusive
breastfeeding to that age would also be beneficial. This conclusion was
translated into a recommendation of exclusive breastfeeding for 6 months,
and while complementary feeding of high-quality should be introduced at
that age, breastfeeding should preferentially be continued beyond the second
year of life. Although some beneficial effects, particularly the prevention of
infections, will be more pronounced in low-income than in high-income
societies, other impacts on mental development and immune functions, for
example, are likely to be important also in high-income societies. It is
therefore clear that the new recommendations will affect the infant feeding
mode on a global level. Moreover, during the last decades it has become
evident that infant nutrition has developmental and health effects which may
last beyond infancy, even into adulthood

Hypertension is a global phenomenon and is common to all human populations with the exception of some primitive communities that live in cultural isolation, for example, in the Amazon
basin. It accounts for up to 6% of adult deaths worldwide. Some reports suggest that as of
203, there are billion people worldwide, who might have hypertension. It is believed that
there is a global epidemic of yet unknown proportions as abnormally elevated blood pressures
are often asymptomatic. Indeed the first diagnosis of hypertension is often made when the
individual presents with a myocardial infarction or a stroke. Hypertension is thus rightly often
referred to as the ‘silent killer’. Yet it was not always thought to be a bad thing. Historically, the
term ‘benign essential hypertension’ reflects the attitudes towards high blood pressure in the
early twentieth century.
