This book critically reviews the evidence surrounding the impact of dietary patterns
and nutrition on brain function and cognitive performance and the mechanisms which
underpin this. The increase in public awareness of the role diet can play in brain
function has been accompanied by a significant development of products, dietary
supplements, functional foods, nutraceuticals, food programs and submissions of
dossiers for health claims and public health recommendations for maintaining brain
function. The area of nutrition–cognition research is an emerging interdisciplinary
field of work that examines the impact of food, nutrients and diet on everyday aspects
of cognitive performance and brain function. It is our hope that this book serves its
purpose: to make available a detailed and innovative scientific summary of nutrition–
cognition research to provide valuable information regarding nutritional and lifestyle
choices for cognitive health.

The chan ging diet of humans through our history is subject to a great deal of conten-
tion. However, what is not in dispute is that our diet has changed signi fic antly in both
a qual it at ive and quant it at ive sense as we have moved through periods as hunter-
gather ers or subsist ence farmers to a situ ation today where we are now consumers
of highly processed and ready- prepared foods. At the same time our patterns of
phys ical activ ity have also changed from the demands of high activ ity neces sary for
survival and strenu ous occu pa tions to those of limited phys ical burdens and sedent ary
occu pa tions or leisure pursuits. For most of history, humans have exper i enced food
short ages, hunger and under- nutri tion as they struggled each day to secure suffi cient

Magnesium is an essential mineral which is required for growth and survival of humans. Since magnesium
is a mineral and not synthesizable, it must be obtained through dietary foods and/or supplements.
Magnesium is found in many sources, primarily whole grains, green leafy vegetables, nuts, and
legumes. Even with many dietary sources, only about one-third of Americans maintain the appropri-
ate dietary intake of magnesium. A very small number of people have drug-induced severe magne-
sium de fi ciency. So major issues reviewed are the bene fi ts of magnesium supplementation to reach
(a) recommended intakes or (b) above -recommended intakes to promote health or treat various
diseases and risk factors.

In Shakespeare’s The Twelfth Night, the story is told of a young woman who became severely
depressed because she was in love with somebody and could not tell anyone about it, not even the
beloved himself. She is described as suffering from a ‘‘green and yellow melancholy,’’ a green
sickness resulting from the teenager having fallen in love. It is a fact that iron deficiency
commonly occurs after a few menstrual periods and imparts a green color to the complexion,
while the yellow is no doubt due to jaundice and liver disease that will often cause a person to
become depressed.

Environmental factors such as suboptimal parental diet can lead to perturbations of in utero
growth and development. In turn, this can have long-lasting negative effects on health in
later life by leading to increased risk and earlier onset of diseases such as type 2 diabetes,
obesity, cardiovascular conditions, cancer, and neurological disorders. In addition, future
generations are at risk of inheriting these conditions through nongenetic mechanisms.
Elucidating the molecular pathways involved in these processes may help in the design of
prevention strategies and may also lead to the identification of biomarkers and potential
drug targets for improved patient management.

The complexity of lipids is one of the most exciting areas in human nutrition. Most
neophytes have actually a simplistic view, reducing them to their energy intake,
enhanced by prevention messages that have demonized them for a long time: fats
make you fat; they “give” cholesterol (even it is ugly and vulgar!), flee! Ideas die
hard, and when they are wrong, they can last for several generations. This shows the
importance of careful scientific discourse, any simplification or overgeneralization
is harmful, as the data changes. Today we know, for example, that the saturated fatty
acids are not a homogenous group, as are the polyunsaturated fatty acids. We know
that the saturated fatty acids are nonessential but useful and that the polyunsatu-
rated fatty acids exert considerable structural and functional features. We know that
omega-3 fatty acids are molecules essential for brain and retina functions very early
in life. No fats, no life

The food we eat has a major effect on our physical health and psychological well-being. An
understanding of the way in which nutrients are metabolised, and hence of the principles
of biochemistry, is essential for an understanding of the scientific basis of what we would
call a prudent or healthy diet.

of this knowledge. This series is intended for researchers in the basic biomedical sci-
ences and clinicians. The potential of such knowledge for healthy aging and disease
prevention warrants further knowledge about how oxidants and antioxidants modu-
late cell and tissue function.

The explosive advances in clinical nutrition knowledge since the publication of the second edition
of this book have made it necessary to make some changes in this new edition, including the
addition of three new chapters. With the completion of the Human Genome Project in 2003 and the
development of new technology, the number of genes of interest in clinical nutrition has expanded.
The new research is expected to greatly advance our knowledge of nutrition and health.

Intravenous (parenteral) nutrition can be lifesaving and is an essential intervention
for those without a functional gastrointestinal tract. Lipids have been in clinical use
as components of intravenous nutrition for over 50 years. They were introduced as a
source of energy and essential fatty acids, undoubtedly two important roles. Initially,
a number of plant seed oils were explored as sources of lipids for use in intravenous
nutrition. Soybean oil was favoured over other candidate oils and, since the 1960s,
emulsions of soybean oil with egg lecithin have been the most widely used. A better
understanding of the metabolic and functional roles of the fatty acid components of
intravenous lipid emulsions has led to a re-consideration of what lipid emulsions
should bring to intravenous nutrition beyond the basic nutritional attributes of energy and essential fatty acids. There is a view that pure soybean oil may not present
an optimal fatty acid composition, being very rich in one fatty acid, i.e. the essential
omega-6 fatty acid linoleic acid. Hence, progressively more complex lipid formulations have been introduced that typically combine soybean oil with one or more other oils. These formulations include mixtures of soybean oil with the so-called ‘medium-chain triglycerides’ or MCTs, which are usually derived from coconut oil or palm
kernel oil, mixtures of soybean oil with olive oil, mixtures of soybean oil, MCTs and
fish oil, and mixtures of soybean oil, MCTs, olive oil and fish oil. A pure fish oil emulsion is also available, as are the so-called ‘structured lipids’, in which the fatty acids
from soybean oil and MCTs have been randomly inter-esterified. These emulsions are
all considered safe and well tolerated, although they may be cleared from the circulation at different rates. The most exciting characteristics of this range of lipid emulsions for intravenous use is that they offer the opportunity to deliver high amounts of
specific fatty acids and that they are likely to possess different functional properties;
in particular, they can influence inflammatory processes, immune responses and hepatic metabolism. The uptake of the different new lipid emulsions into routine clinical care has been varied according to patient type and geography, but these emulsions
have been subject to considerable research. Relevant applications include children
and adults who require intravenous nutrition because of short bowel syndrome, premature infants, those destined to undergo elective surgery, post-surgical patients, and
the critically ill. These varied patient groups may benefit in different ways from the

new lipid emulsions, for example, some could benefit from a better balanced supply
of fatty acids for maintenance of organ function and others could benefit from the
ability of some fatty acids, such as the long-chain omega-3 fatty acids found in fish
oil, to modulate inflammation and immune responses. The new lipid emulsions may
also offer the opportunity to deliver high amounts of specific functional fatty acids
in acute settings such as after severe head injury or myocardial infarction. This book
brings together expert authors to provide state-of-the-art reviews of different nutritional, technological, and clinical aspects of the lipid emulsions designed for intravenous nutrition. It is our belief that the articles herein will provide the reader with a
broad range of relevant and up-to-date information on the covered topics. In our view,
these articles will appeal equally to basic scientists, clinical researchers and clinical
practitioners and will serve to provide significant advances in the knowledge and understanding of this field. Of course, this is a moving field, with new studies being
published regularly; nevertheless, these articles will remain a valuable resource to
understand the background on newly emergent research in this exciting field.

As the epidemic of diabetes continues to expand in parallel
with the rapid spread of obesity, healthcare providers strive
to find interventions to reduce the morbidity, mortality, and
rising costs associated with this devastating disease, which
ravages both the micro- and the macrovasculature. Although
the increase in incidence of type 2 diabetes may be attributed
to the expanding girth of the population coupled with a lack of
physical activity, the marked increase in the incidence of type 1
diabetes remains unexplained. Our knowledge of the cellular,
biochemical, and molecular etiology of impaired insulin action
and beta-cell failure has expanded enormously, but the genetic
basis of both type 1 and type 2 diabetes and their associated
complications is still by and large undefined. Despite the introduction of multiple new classes of antidiabetic agents for the
treatment of type 2 diabetes, and newer insulin preparations,
insulin delivery systems, and glucose-sensing devices for the
management of type 1 diabetes, glycemic control is suboptimal
in approximately half of all diabetic patients and the excess
risk for macrovascular complications is largely unexplained.
In many parts of the world, these treatment advances are not
available and instituting behavioral modification programs at
the societal and individual level is proving to be inadequate
in curbing the growing epidemic of obesity. Whether the
introduction of novel weight loss medications will stem the tide
of obesity remains to be determined.

In the current environment, cardiovascular disease continues to
be one of the causes – if not the leading cause – of mortality
worldwide. Therefore, it is important to have publications and
books, which provide appropriate perspective on the clinical
management of these conditions. What makes this book timely
and clinically relevant is linking it to the ongoing worldwide
epidemic of diabetes, which appears to be a major factor for
retarding the rate of decline in cardiovascular disease

This volume is a result of the certainty that we can learn from each
other. Especially, there is a great deal to be learned by studying the way
people from different cultures and from different countries respond to a
socially defined individual behavior, such as adolescent pregnancy. This
volume provides a multitude of views on adolescent pregnancy that can
help our thinking move from the oversimplified constructs based on our
own cultural perspective to a construct that is built upon a foundation
of biological science (e.g., a knowledge of child development, and sexual
and reproductive development), issues common to all adolescents,
particularly girls. These differences between the way people from various
countries respond to adolescent pregnancy, as will be observed in these
chapters, is the result of religious and cultural beliefs specific to
individual groups and individual countries.

Since the Nutrition Care Process and Model development, the Academy of Nutrition and Dietetics has released several
publications related to the Nutrition Care Process (NCP). The original article published in 2003 described the complete
Nutrition Care Process (1). This article was updated in 2007-2008 by a subcommittee of the Nutrition Care Process
Standardized Language (NCP/SL) Committee chaired by Annalynn Skipper, PhD, RD. Part one of the updated article
was published in the July 2008 issue of the Journal of the American Dietetic Association (2). The second part of the
article was published in August 2008 in the Journal of the American Dietetic Association (3).
